Referral Form

When a client would like to initiate a rehabilitation therapy program for their pet, my next step is to make a referral request from the primary or specialty veterinarian. This is important to:

1) Establish a line of communication with the veterinarian so we can work together for the benefit of the pet

2) Get information about the pet's history, diagnosis, and what treatments have already been done

3) Discuss details that may be unique to the case such as with post-surgical cases, geriatric dogs, dogs with multiple conditions, etc.

Click HERE to download and print the referral form. The form can be scanned and submitted to sophia@onpointanimalrehab.com along with any relevant medical records. It can also be given to the client to bring to their first rehab appointment.
